What is it? A form used to withdraw money from an H&R Block Easy IRA.
What is it used for? To take a distribution from an H&R Block Easy IRA.

The following are the fields that must be completed before processing: account number (or SSN), name, distribution amount, method of payment (include new address if different from account), signature and date.
