I don't want to file extension, but I didn't receive all the Forms 1099-R I need to file my return. Since I know the amounts and don't want to file an extension, what can I do regarding the missing 1099-R?

Contact your payers to request your Forms 1099-R. If they’ve sent the forms, but you haven’t received them, they might need to resend the forms.

If you still don’t receive your Forms 1099-R within a reasonable amount of time, you have two options:

  • Call the IRS at 800-829-1040. Be prepared to provide your:
    • Name and address
    • Phone number
    • Social Security number (SSN)
    • Payer’s name, address, and phone number
  • File Form 4852: Substitute for Form W-2, Wage and Tax Statement, or Form1099-R.

If you file Form 4852, attach it to your return. Write an explanation detailing your efforts to obtain your late 1099-R Forms.
