I have a question about Form 1099-R Box 7 — How can I tell from my Form 1099-R if my distribution came from a SEP, IRA, or SIMPLE plan?

On your Form 1099-R Box 7 there’s a checkbox labeled “IRA / SEP / SIMPLE.” If it’s checked, then you have one of these distributions. You can contact your plan administrator for more detailed information.
