See the &#8220;Instructions for Submitting this Form&#8221; in Form 14039, for information on submitting it by mail or fax.<\/p>\n<h2>More from H&amp;R Block<\/h2>\n<p>IRS Form 14039 is an identity theft affidavit that you send the IRS if your personal information has been stolen. After the IRS processes your identity theft affidavit, the IRS will place an <a href=\"https:\/\/www.hrblock.com\/tax-center\/irs\/audits-and-tax-notices\/tax-dictionary-identity-theft-indicator\/\">identity theft indicator<\/a>\u00a0on your tax account to show that your personal information was stolen and to help prevent future tax refund fraud.<\/p>\n<p>The IRS will also send you an Identity Protection Personal Identification Number (IP PIN) in December or January. The IP PIN is extra confirmation of your identity when you file your tax return. It helps prevent another individual from filing a return using your Social Security Number.
